The teacher request form, field by field

Every step and field of Put a teacher forward: what is required, the limits and formats, uploading documents, referees, and what the error messages mean.

For:Campuses13 min read15 sectionsUpdated 2 October 2026

Put a teacher forward is a five-step form. It sends NSDP the candidate's details so NSDP can verify them, usually interview the candidate, and decide. This guide explains every field and the rules the form checks. For the process as a whole, see Putting a teacher forward from your campus.

How the form works

  • Fields marked with a red * are required. Everything else is optional, but the more NSDP has, the faster it can verify the candidate.
  • Continue checks the step you are on. If something needs fixing, the field is highlighted with a message under it and the cursor jumps to it; you stay on the step until it is fixed.
  • The step list (a rail on wide screens, All steps on phones) shows your progress. You can jump back to any step you have already reached; a step with problems shows a red mark and how many fields to fix.
  • Earlier steps stay editable until you send. Nothing reaches NSDP until you press Put this teacher forward on the last step.
  • Longer fields show a counter such as 120 / 300. It turns amber near the limit, and typing stops at the limit.
  • The form is not saved as a draft. If you close the tab, reload or leave the page, what you typed is lost.

Step 1: Candidate

Who the candidate is and how NSDP can reach them. NSDP identifies a candidate by CNIC and email, and the email becomes the teacher's sign-in once they are approved.

Identity

First nameRequiredUp to 80 characters
As written on the candidate's CNIC.
Last nameRequiredUp to 80 characters
As written on the candidate's CNIC.
CNICRequired13 digits
The dashes are added for you as you type, e.g. 35202-1234567-1. See The CNIC field below.
GenderDefault: Prefer not to say
Male, Female or Other.
Date of birthNot in the future
Pick the date from the calendar.
NationalityUp to 80 characters
Choose from the list or type it.

Contact

EmailRequiredUp to 160 characters
The candidate's own address. NSDP uses it to arrange the interview, and it becomes the teacher's login when approved. It can't already belong to an account on the portal.
PhoneRequiredSee **The phone fields** below
Mobile or landline. Pick the country code, then type the number without the leading 0.
Alternate phone
A second number NSDP can try. Same rules as Phone.

Address

Province / territoryRequired
Where the candidate lives. Changing it clears a listed city from another province.
CityRequiredUp to 80 characters
Search the list for the chosen province. Not listed? Type it, or pick Other city or town — type it. Picking a city first fills in its province.
Street addressUp to 300 characters
House, street, area.
Postal codeUp to 12 characters
Pakistani postal codes are 5 digits, e.g. 54000.

The CNIC field

  • Type the 13 digits of the candidate's CNIC. The form adds the dashes itself (35202-1234567-1), and anything that isn't a digit is ignored.
  • The form checks the digit count and tells you how many you entered, e.g. CNIC must be 13 digits (12 entered).
  • The CNIC is how NSDP recognises the same person across requests. If someone with this CNIC (or this email) already has a request open with NSDP, the request is refused. See Common errors below.
  • When resubmitting or revising a request you can correct a mistyped CNIC; the same duplicate checks run again.

The phone fields

Choose the country from the code picker (Pakistan +92 is the default), then type the number without the leading 0. The number is stored with its country code, e.g. +92 300 1234567.

CountryDigits after the codeExample
Pakistan +92, mobile (starts with 3)10300 1234567
Pakistan +92, landline9 to 1042 1234567
United Arab Emirates +9718 to 9
Saudi Arabia +9668 to 9
Qatar +9748
Oman +9688
United Kingdom +449 to 10
United States / Canada +110

Step 2: Qualifications & experience

What the candidate has studied and where they have worked. NSDP checks these against the documents you attach.

Fields on this form

Highest qualificationRequired
Matriculation, Intermediate, Bachelor's degree, Master's degree, PhD or Other. Their highest completed qualification.
Field of studyUp to 160 characters
Choose a suggestion or type it, e.g. Computer Science.

The professional summary. See below.

The professional summary

  • One or two sentences on who the candidate is as a teacher, e.g. Full-stack developer with 6 years in industry, 3 of them teaching web development to beginners.
  • It is optional, up to 300 characters.
  • It heads the candidate's card on your Teachers page and is included when you use Copy profile. NSDP's reviewers see it too.
  • Once the teacher is approved it stays on their profile in your list of approved teachers.

Step 3: Subjects & availability

What the candidate can teach, how, and when they are usually free. NSDP matches approved teachers to batches by their subjects and teaching mode.

Fields on this form

Expertise areasRequired1 to 20 subjects, up to 80 characters each
Type a subject and press Enter or a comma after each one. The same subject is only added once.
Teaching modeRequired
Online only (live or recorded classes over the internet), In person at the campus (classes in your classrooms and labs) or Both in person and online (can take either kind of batch).
Teaching methodologyUp to 3,000 characters
Class style, tools and how they assess learners. Seen by the NSDP reviewer.
Available days
Tap the days they can teach, Monday to Sunday.
Available time slotsUp to 20 slots
Tap the usual slots or add a custom range. See below.

Availability: days and time slots

  • Availability is optional, but it helps NSDP plan batches around the candidate.
  • The four ready-made slots are Morning (9am–12pm), Afternoon (12pm–3pm), Late afternoon (3pm–6pm) and Evening (6pm–9pm). Tap one again to remove it.
  • For anything else, choose a Start and End time (half-hour steps between 6am and 10pm) and add it as a custom slot, e.g. 4pm–7:30pm.
  • The end time must be after the start time, and the same slot can't be added twice.

Step 4: Documents & references

Supporting links, documents and people NSDP may call about the candidate. Everything on this step is optional, but CNIC, degree and experience letters help NSDP verify the candidate faster.

Online profiles

LinkedIn profileFull link starting with `https://`
Optional.
GitHub profileFull link starting with `https://`
Optional.
Portfolio or websiteFull link starting with `https://`
Optional.
Introduction videoFull link starting with `https://`

Each document (Add a document, up to 15)

Document typeRequiredDefault: CNIC — front
CNIC — front, CNIC — back, Degree or certificate, Experience letter, Photograph, CV or Other document.
File nameUp to 200 characters
A label such as BSc transcript. Filled in from the file name when you upload, if empty.
Upload a fileRequiredPDF, JPG, PNG, WebP or Word; max 10 MB
Uploads the file and attaches it to the document row. Replace the file uploads a different one.
File
Shows Uploaded with the file name once a file is attached, or No file uploaded yet.
  • Every document needs a file. Press Upload a file on the row and choose it from your device; the portal stores it and NSDP opens it from there. Links to files stored elsewhere (Google Drive, OneDrive and similar) are not accepted.
  • A document row you added but left without a file blocks sending. Upload the file or press Remove on that row.
  • Every document except the Photograph is stored privately: only your campus and NSDP staff can open it, through View on the row while signed in. It has no public link and never appears in your media. The Photograph becomes the teacher's public portrait.
  • Because a photograph is public and the other types are private, changing a row between Photograph and another type clears its file: upload it again.

Uploading files

  • Press Upload a file on a document row and choose the file. Accepted: PDF, JPG, PNG, WebP, and Word (.doc, .docx), up to 10 MB each. The button shows the progress.
  • One file per document row. Add a row for each document (up to 15).
  • For the candidate's profile photo, add a document with the type Photograph. The first photograph is used as the candidate's picture on their request card.
  • Uploaded files are stored by the portal; nobody has to copy or paste anything.

Referees

Each reference (Add a reference, up to 5)

NameRequiredUp to 120 characters
The referee's full name.
ContactRequiredUp to 160 characters
A phone number or email NSDP can use.
RelationshipUp to 120 characters
Choose a suggestion (e.g. Former line manager, Head of department, Principal) or type it.

Step 5: Review & send

  1. Step 1: Check the summary

Each step is summarised with an Edit link. Empty optional fields read Not provided.

  2. Step 2: Fix anything flagged

If a step still has a problem, a red box lists it with a Fix link that takes you to the field. The send button stays disabled until everything required is in place.

  3. Step 3: Put this teacher forward

When the box says Everything required is in place, press the button. The request goes to NSDP.

  4. Step 4: Keep the reference

The confirmation shows a reference such as TUT-7KQ2-M4XD. Use View requests to follow it, or Put forward another to start again with an empty form.

FAQ

Can I save the form and finish it later?

No. The form isn't saved as a draft, so have the candidate's details and documents ready before you start and send it in one sitting.

Whose email should I enter?

The candidate's own email address. It becomes their login when NSDP approves them, and NSDP sends their set-password link to it. Don't use the campus email.

Do we have to attach documents?

No, documents are optional. Without them NSDP may ask for more information before it can verify the candidate, which slows things down.

We made a mistake after sending. What now?

You can't edit a request NSDP has. Contact NSDP with the reference. If NSDP asks for more information you can correct it then.

It says the person is already being processed, but we never put them forward.

Another request for the same CNIC or email is open, for example from the NSDP website. Contact NSDP administration with the candidate's details.
